This proforma (completion time approx 10 minutes) seeks to gather opinions about a range of issues related to the practices and performance of XX.

It is being issued to Executives in businesses and organisations that are customers of XX. The information gathered will be analysed used to identify opportunities to enhance the value added of XX to it's constituencies.
